ADSL modem - Cisco model 677 SOHO/TELECONMMUTER configuratio

I run an iMac with mac OS 9.2.2 and need help to configure the cisco 677 modem for DSL with the TPC/IP control panel. I need the settings.

UserAgent: Mozilla/4.0 (compatible; MSIE 5.0; Mac_PowerPC)
